PATIENT RIGHTS
As a patient of Dr Brooke Quigley at Ochre Medical Centre Wollongong, you have the right to:
Access
  • Healthcare services and treatment that meets your needs
Safety
  • Receive safe and high-quality health care that meets national standards
  • Be cared for in an environment that is safe and makes you feel safe
Respect
  • Be treated as an individual, and with dignity and respect
  • Have your culture, identity, beliefs, and choices recognised and respect
Partnership
  • Ask questions and be involved in open and honest communication
  • Make decisions with your healthcare provider, to the extent that you choose and are able
  • Include the people that you want in planning and decision-making
Information
  • Clear information about your condition, the possible benefits and risks of different tests and treatments, so you can give your informed consent
  • Receive information about services, waiting times and costs
  • Be given assistance, when you need it, to help you understand and use health information
  • Access to your health information
  • Be told if something has gone wrong during your health care, how it happened, how it may affect you and what is being done to make care safe
Privacy
  • Have your personal privacy respected
  • Have information about you and your health kept secure and confidential
Give Feedback
  • Provide feedback or make a complaint without it affecting the way that you are treated
  • Have your concerns addressed in a transparent and timely way
  • Share your experience and participate to improve the quality of care and health services

PATIENT RESPONSIBILITIES
As a patient of Dr Brooke Quigley at Ochre Medical Centre Wollongong, you are responsible for:
  • Telling us about your illnesses, hospital visits, symptoms, medications, allergies and other health matters
  • Telling us about any religious or cultural beliefs and requirements
  • Asking questions and talking to your family before making any decisions about your health care, if relevant
  • Following your healthcare provider’s instructions regarding your treatment and care
  • Being aware that your care may need to be transferred to another specialist or hospital if required
  • Talking to your doctor if your condition changes while on a waiting list for treatment
  • Being on time for appointments
  • Letting us know ahead of time if you need to cancel or reschedule
  • Notifying us if your contact details change
  • Treating all people you meet at Ochre Medical Centre Wollongong (including staff, students, volunteers, patients/clients and their families) with care, dignity and consideration
  • Respecting the confidentiality and privacy of others.
 
Threatening, aggressive or violent language and behaviour towards Dr Quigley or Ochre Medical Centre Wollongong staff will not be tolerated and Dr Quigley reserves the right to take action if necessary.
